Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County.

We work to coordinate services and support systems across a network of state and local organizations and help empower our community by providing access to information and support resources. We assist you in gaining access to important support services provided directly to you by Sourcewise, such as Health Insurance Counseling, Meals on Wheels, Case Management, and important information on other crucial services available to you throughout Santa Clara County.
